The Siouxland Community Health Foundation will host an afternoon of golf at the Sioux City Country Club on Monday, July 13. In its fourth year, the event welcomes our friends and professional partners throughout the Siouxland community and will feature an 18-hole scramble with various pin prizes throughout the tournament. A bag lunch will be available prior to the noon shotgun start, as well as dinner during the awards presentation.
 
The Siouxland Community Health Foundation would like to extend an invitation to you to register a foursome team or consider a sponsorship in support of the event. All proceeds from the event will support programs and services for our patients at Siouxland Community Health Center. Furthermore, your partnership advances our mission to serve the community and bring healing through our commitment to care. Enclosed is a team registration and sponsor commitment form complete with event details. You may also visit our events page at slandchc.com to complete online. All team and sponsor registrations are due by Friday, July 10, 2020.
